Dr. Afzali is a
graduate of the University of Missouri in Kansas
City Dental School, one of the best in the
country.She received the Exceptional Academic
Achievement Scholarship and Dean’s Academic
Honor Roll for high academic performance. She
also received honors in the periodontal program
and was selected for Early Completion of Degree
Requirements and received Acknowledgment for
Significant Contribution made in the Advancement
of Scientific Knowledge and its Application to
Patient Care.
She has been practicing as a dentist since 1990
and has been through many post graduate courses
in different fields of dentistry, such as
comprehensive training in the area of Diagnosis
and Treatment Planning, Treatment of
Tempomandibular Joint Disorders, Advances in
Therapy for Periodontics Patient Soft tissue
Management, dental Implants, esthetic dentistry
and Training in Functional Orthodontic
Appliances.
Her emphasis on patient comfort as well as
keeping up-to-date with the latest advancements
in dentistry enable her to provide her patients
with the best quality of care in the field.
Dr. Afzali has a wide range of experience in
different areas of dentistry, from cosmetic and
restorative dentistry to surgical treatments.
Her advanced knowledge in non-invasive,
non-surgical, and preventative periodontal (
gum) treatment, has saved thousands of her
patients from complicated surgical procedures.
Her approach to your treatment starts with
patiently listening to you. After a complete and
thorough examination, she reviews and discusses
the diagnosis and treatment options with you.
She is happily married with a teenage daughter
and son. Besides humanitarian activities, she
enjoys gardening, growing orchids, reading and
playing the piano.
Dr. Afzali founded Glenlake Dental Care about
twenty years ago with the purpose of helping to
raise dental awareness to the community.

Dr. Eric Lim graduated from Northwestern University Dental School in
Chicago, Illinois in 1999 with a degree in Doctor of Dental Surgery where he
received the Gilbert J. Brinsden Award in Fixed Prosthodontics for
excellence in the area of crowns and bridges. He continued his education in
the following year by completing a specialized dental training residency at
Loyola University Medical Center in Maywood, Illinois. Prior to starting his
dental education, he received a Bachelor of Science degree in Biology from
Duke University in Durham, North Carolina. As a member of the Academy of
General Dentistry, he has completed nearly 500 hours of continuing
education.
He enjoys practicing all aspects of dentistry; however he has gained
extensive interest and experience in the areas of full mouth rehabilitation,
implants, and cosmetic dentistry.
Dr. Lim has been practicing as a general dentist at Glenlake Dental Care
since year 2000. High professionalism and unique personal care best describe
this doctor. His goal is to provide the best dental care to each patient
with compassion. "I always try to create a comfortable
and pleasant experience for my patients at each appointment."
He is happily married and has one son. Besides his passion for
dentistry, he enjoys taking trips with his family, being active in the local
church, playing the guitar, playing softball, and golfing.
Dr. John Sullivan earned a Doctor of Dental Surgery
degree from the University of Illinois College of Dentistry in 1980. He has
enjoyed practicing dentistry for over 28 years, treating patients of all
ages and performing a wide variety of dental procedures to accommodate all
types of conditions.
Dr. Sullivan has been keeping his knowledge up to date by taking continuing
education training in many areas of dentistry. He believes it is very
important to continually educate himself in new and improved procedures,
tools, and techniques, so he can provide his
patients with the highest quality of dentistry in a comfortable setting.
Dr. Sullivan believes that prevention is the key to healthy teeth and gums, so he has enhanced his knowledge in the area of
micro-dentistry and esthetic conservative restorations (porcelain inlays & onlays, composite bonding, and veneers). He has received training in dental
implants, crowns, bridges, root canal treatment, implant supported dentures,
Lumineers, and professional teeth whitening (Zoom, Britesmile and Rembrandt
systems). He also has received extensive training and certification in soft
tissue management to keep gums healthy for a lifetime. By listening to his
patients and understanding what they would like to accomplish in regard to
their teeth and gums, Dr.
Sullivan creates a treatment plan that is ideal for the patient.
In his spare time, Dr. Sullivan enjoys running,
volunteering for church projects, and
spending time with his two teenage sons.
